(Comment in: N Engl J Med. 1989 Apr 13;320(15):1010-1.)

The discussion presents several important points. Both the black and white women had equal opportunity to use prenatal care; however, the black women used the Kaiser prenatal services less than their white counterparts. The prepaid nature of the plan removes financial barriers. The authors conjecture that other barriers that may prevent the black women from using the available care may involve issues surrounding transportation-related problems, child-care arrangements, inflexible employment schedule and lower level of satisfaction with the prenatal care among the black women than among the white women. Furthermore, the authors recommend that further research should explore the differences in cultural perception of the value of prenatal care and how it may manifest into dissimilar patterns of utilization.
